The full name, affiliation, and email address of your chosen representative to the Shareholder Consortium. If not, not in "Additional Comments" that the name of a rep is forthcoming.

Note regarding payment methods: By default, payment will be made using PayPal (which can pay via a PayPal account or any credit card). Use the offline payment method (at bottom of the form) if you prefer to be invoiced. Use the Additional Info field to indicate if the invoice should be addressed to someone else in your organization.
Note that no organization or program that wishes to become a JAFSCD Shareholder will be turned away for lack of ability to purchase a share; contact Duncan Hilchey to discuss options.
